Successful Emergency Treatment of a Diabetes Case with Blood Glucose Reaching 90.6 mmol/L

From a critical condition, a patient with severe complications from diabetes was successfully treated by the team of doctors at the Emergency Intensive Care Unit of Hoan Hao Kei Mei Kai General Hospital in Di An.
The patient was admitted with extremely high blood glucose, septic shock, and acute kidney injury.

Recently, the Emergency Intensive Care Unit received a 40-year-old female patient with a long history of type 2 diabetes and hypertension, both poorly controlled.

Condition upon admission:

  • The patient was lethargic, had a rapid pulse, and low blood pressure.

  • Capillary blood glucose test at the site showed “HI” (exceeding the device's limit).

  • Laboratory blood glucose result: 90.6 mmol/L – a critically dangerous level that could rapidly lead to death.

Initial diagnosis:
Multiple life-threatening complications

An urgent consultation between Emergency and Intensive Care doctors determined the patient had the following severe complications:

  • Septic shock originating from the gastrointestinal tract

  • Acute complications of type 2 diabetes: hyperosmolar hyperglycemic state, diabetic ketoacidosis, and lactic acidosis

  • Acute kidney injury

  • Hypernatremia, hypertension, and mixed dyslipidemia

Aggressive treatment protocol in the Intensive Care Unit
The patient was immediately transferred to the ICU and given emergency treatment, including:

  • Continuous insulin infusion to control blood glucose

  • Combination of broad-spectrum antibiotics to combat infection

  • Noradrenaline to maintain blood pressure

  • Close monitoring of fluid balance, electrolytes, kidney function, and vital signs

Treatment outcome: Stabilized after 7 days, safely discharged
After 2 days of intensive care:

  • The patient became more alert and was able to eat

  • Blood glucose levels gradually decreased

  • Kidney function showed marked improvement

  • Vasopressors were discontinued, and insulin therapy switched to subcutaneous injections

By day 7, the patient had fully stabilized and was discharged in the joy of her family and the pride of the entire treatment team.

Do not underestimate diabetes
This case is a real-life warning to patients with type 2 diabetes who do not manage or discontinue treatment. Once complications arise, the risk becomes extremely high and can be fatal in a very short time.

  • Monitor blood glucose levels daily

  • Follow your doctor’s treatment plan

  • Eat a balanced diet, reduce starch, sugar, and unhealthy fats

  • Have regular check-ups to detect complications early
